Did You Know:

  • For the majority of insurances, or if you are paying a self-pay rate, we can set you up for an evaluation and treat you for up to 30 days without a referral. Within 30 days, we will contact your physician and have them approve the plan of care outlined by your COSPT clinician.​
  • If you are covered under Medicare, workers’ compensation, or automobile insurance, then you will need a referral from a physician before you can begin treatment.
  • If you already have a referral, you can either bring it with you to your first visit or contact your physician and have them fax it to us at 850-877-2917.

Wow. When I say 5 stars, I mean just the absolute best about these doctors that I can say. Working with Halley Miller has been an absolute life-changer. She had already demonstrated that she was incredibly good at observing how my body moved and listened well in how i described my pain. Her communication skills are excellent, and my pain has been cut significantly with each progressing step in my program. Even with my restrictive and sometimes volatile schedule, she has been working diligently and patiently with me to deliver me to a healthier state of life. As I do my exercises today, I just can’t thank her enough. I couldn’t have asked for anyone more qualified to restore this sweet spine ‘o mine🙏🏽🎸
